CNS, Head & Neck, Thoracic Tumors
Care Before the Procedure
Diagnostic Workup:
- Imaging: MRI/MRS for CNS tumors; CT/PET-CT for head/neck and thoracic tumors.
- Biopsy: Stereotactic or endoscopic biopsy (e.g., bronchoscopy for lung tumors; transoral robotic surgery for head/neck tumors).
- Molecular Testing: EGFR, ALK, PD-L1 for lung cancer; IDH mutation, MGMT methylation for glioblastoma; HPV status for oropharyngeal cancer.
- Preparative Care: Nutritional support (feeding tubes if needed for head/neck tumors). Pulmonary rehabilitation for thoracic tumors. Speech/swallow evaluation and prehabilitation. Fertility preservation discussions for young patients. Psychological support and advanced care planning.
Care During the Procedure
Treatment Modalities:
- Surgery: Craniotomy for CNS tumors; transoral laser microsurgery for head/neck tumors; lobectomy/pneumonectomy for lung cancer.
Emphasis on maximal safe resection. - Radiation: Intensity-modulated radiation therapy (IMRT) or proton therapy to spare adjacent organs.
Stereotactic radiosurgery (e.g., Gamma Knife) for brain metastases. - Systemic Therapy: Chemotherapy: Temozolomide for glioblastoma; platinum-based regimens for head/neck/thoracic cancers.
- Targeted Therapy: Osimertinib for EGFR-mutant lung cancer; trastuzumab for HER2+ tumors.
Immunotherapy: Checkpoint inhibitors (e.g., pembrolizumab) for PD-L1+ tumors. Supportive Care - During Treatment: Antiseizure medications (CNS tumors). Nutritional support via PEG tubes (head/neck tumors). Management of esophagitis, dermatitis, or pneumonitis from radiation.
Care After the Procedure
Survivorship Care:
- Regular imaging (MRI/CT) and clinical exams to monitor for recurrence. Long-term management of treatment-related effects:
- CNS: Cognitive rehabilitation, seizure control.
- Head & Neck: Speech/swallow therapy, dental rehabilitation.
- Thoracic: Pulmonary function monitoring, cardiac health.
- Lifestyle Modifications:
Smoking cessation programs. Dietary adjustments for swallowing difficulties.
Physical therapy to address functional deficits. - Psychosocial Support:
Support groups for specific cancer types (e.g., brain tumor networks).
- Counseling for anxiety/depression related to functional changes.
- Palliative and Hospice Care: Early integration for symptom control (e.g., pain, dyspnea) in advanced cases.
- Focus on quality of life when cure is not achievable.
